Ketone Strips Measure Something, Just Not What You Want

A colour change proves you are in ketosis, which is a metabolic state rather than an achievement. What it does not prove is that you are losing any of the fat you started the diet for.

What ketones are and when they appear

When carbohydrate intake drops far enough, the liver converts fatty acids into ketone bodies that most tissues can burn instead. This is a normal metabolic route, used during fasting and prolonged exercise as well as on a very low carbohydrate diet. It is not a special state the body was designed to be pushed into, and it is not evidence of anything being cleansed.

Ketone production begins within a day or two of sufficiently low carbohydrate intake in most people. The presence of ketones therefore confirms low carbohydrate intake and confirms nothing else.

What the strip is actually detecting

Urine strips react with one particular ketone body, changing colour in proportion to how much of it is being excreted. Excretion is waste, so a strong reading indicates the body is producing more than it is using and discarding the surplus.

As adaptation proceeds over weeks, tissues use ketones more efficiently and less appears in urine, so the reading fades. People frequently interpret that fading as falling out of ketosis and eat even less carbohydrate in response. Hydration alone changes urine concentration substantially, which makes the reading noisy in both directions.

The step the diet culture skips

Fat loss requires taking in less energy than you use, and ketosis is a fuel-switching state rather than an energy state. It is entirely possible to be firmly in ketosis while eating enough fat to maintain or gain weight.

Against the trial data, very low carbohydrate diets often do reduce intake, because they eliminate whole categories of easily eaten food and increase satiety. That is a genuine mechanism and it is a behavioural one rather than a metabolic advantage of the state itself. The strip has become the measurable proxy for a goal it does not measure, which is the classic surrogate endpoint trap.

Products that raise the number directly

Exogenous ketone drinks and salts raise blood and urine ketones within an hour, without any change in diet. They therefore produce the reading someone is chasing while doing nothing about energy intake at all. They may in fact reduce the body own ketone production temporarily, since the liver responds to what is already circulating.

Selling a product that improves the measurement rather than the outcome is a recognisable commercial pattern.

Trials comparing low carbohydrate against other approaches generally show an early advantage that narrows considerably over longer follow-up. The early difference is heavily influenced by glycogen and its associated water, which is weight rather than fat.

For blood sugar control specifically, carbohydrate restriction has a more direct and better-supported effect worth taking seriously. Anyone on insulin or on glucose-lowering medicines must involve their prescriber before restricting carbohydrate, because doses may need changing.

Two states with similar names

Nutritional ketosis and diabetic ketoacidosis are different conditions, and confusing them causes needless alarm in one direction and complacency in the other. Ketoacidosis involves much higher ketone levels together with high blood sugar and acidification of the blood, and it is a medical emergency. It occurs mainly in type 1 diabetes and occasionally with certain diabetes medicines even when blood sugar looks normal.

Vomiting, deep rapid breathing, abdominal pain and confusion in someone with diabetes need emergency assessment rather than a strip. Anyone with diabetes considering carbohydrate restriction should be doing it with their diabetes team, not from an internet protocol.

The strip tells you what fuel you are burning, not whether you are running a deficit. A product that only moves the number has told you what it is for.

An extraordinary mechanism needs better evidence than testimonials, and usually has less.
